"Highly qualified RMHN with extensive experience in the care of individuals with various mental health conditions, including schizophrenia, dementia, depression, personality disorders, eating disorders, and community care. In the past, I have worked in medium secure units. At Dene Hospital, I have supported clients with personality disorders, chronic mental illnesses, learning disabilities, dementia, adolescent drug abuse and alcohol use disorders, as well as those with dual diagnoses requiring care due to self-neglect and difficulties managing their activities of daily living. I have worked in an acute ward, admitting patients from the community who, due to non-compliance with their medications, relapsed and became unwell, necessitating acute admission for support. On a day-to-day basis, while working at Dene as a senior nurse, each ward had 20 patients, and I carried out the following duties: • Established therapeutic relationships with clients by actively listening, building trust and rapport, providing support, displaying empathy, and being non-judgemental and approachable. • Conducted daily risk and mental state assessments to promote a safe environment and monitor client progress. • Maintained continuity of care by ensuring comprehensive handover of responsibilities to incoming staff and being in charge of the shifts most of the time, ensuring this was done appropriately and in detail. • Responded to crisis situations and took appropriate action, utilising effective communication, counselling, de-escalation techniques, harm minimisation, and negotiation skills. • Dispensed and administered medication, monitoring efficacy and side effects. • Collaborated with the multidisciplinary team to ensure optimal client care, including participation in team meetings to identify strategies for improving patient care. • Ensured clients' management plans were updated and adhered to, providing client-centred nursing care. • Ensured clients who were detained in hospital were advised of their rights to empower them through understanding. • Acted as the primary nurse for individual patients, establishing close support for several clients, which enabled me to know them well. • Coordinated shifts daily, maintaining safety for patients, staff, and the public. WORTHING HOSPITAL: After retiring from Dene Hospital, I joined a specialised team at Worthing General Hospital to support them in managing mentally disturbed patients with dual diagnoses who were hospitalised due to physical illnesses and placed on level 1 to minimise any disruption of care for other patients in the hospital. My main duties in this hospital included keeping patients and the public safe, supporting patients in distress, engaging effectively with them, updating general nurses and doctors on patient progress daily, and documenting this in the multidisciplinary notes while employing de-escalation techniques. I assisted in calming disturbed patients and advised doctors when necessary to tranquillise them and involve the police. I supported these patients, and when they were well enough, I escorted them safely to their specialised units, providing comprehensive handovers.
