In the past I have worked in medium secure units. In the Dene hospital I have worked with clients with personality disorder, chronic mental illnesses, learning disability, Dementia, young adolescence drug abuse and alcohol offenders, and those with dual diagnosis who are being cared for due to self-neglect and cannot manage to attend to their activities of daily living. I have worked in acute ward admitting patients from the community who because of their non-compliance to their medications relapsed and consequently got very unwell and needed acute admission to be supported back on their feet. On day today basis while working at this hospital as a senior nurse ( DENE) each ward had 20 patients and carried out the following duties, • ➢ Established therapeutic relationship with clients by actively listening, building trust and rapport, support, displaying empathy, being non-judgemental and approachable. • Conducted risk and mental state assessment on a daily basis to promote a safe environment as well as monitor client progress. • Maintained continuity of care by ensuring comprehensive handover of responsibilities to incoming staff and being in charge of the shifts most of the time. I ensured that this was done appropriately and in detail. • Responding to crisis situations and taking appropriate action. This is done by utilising effective communication, counselling, de-escalation techniques, harm minimisation and negotiating skills. • Dispensing and administration of medication, and monitoring efficacy and side effects. • Collaboration with the multidisciplinary team to ensure optimal client care including participation in team meetings to identify strategies to improve patient care. • Ensuring client’s management plans are updated and are adhered to, to provide a client centred nursing care. • Ensuring clients who are detained in hospital have been advised of their rights in order to empower them by facilitating understanding. • Primary nursing for individual patients; became primary nurse for number of clients and making me know my patients well and be able to support them even more closely. • Coordinating shifts on daily basis and maintaining safety for patients, staff and public. WORTHING HOSPITAL; After retiring from the DENE hospital, I joined a specialised team in Worthing general hospital to support them by managing mentally disturbed patients with dual diagnosis who are hospitalised due to physical illnesses and are placed on levels 1. 1. to minimise any disruption of care to any patients in the hospital. My main duties in this hospital were to:Keep patient and public safe, support patient when in distress, engage well with patient, update general nurses and doctors on the progress the patient is making on day to day basis, and documenting the same in the multi-disciplinary notes, using de-escalation technique. I participate in assisting with calming down the disturbed patient and when necessary advise doctors on when necessary to tranquillise patient as well as getting police involved. I support these patients and when well enough I escort them safely to their specialised units and giving out comprehensive handover. Below are some of the major skills I posses. • I consistently identify myself as an accountable practitioner. • I poses a friendly and caring approach, I am genuine, respectful to others with a sense of humour. • I always adopt assessment, planning, acting and review while carrying out justified care plans for patients. • I fully understand risk assessment and risk management. • I am aware of NMC, CQC guidelines and apply them while carrying out day to day duties in the ward especially while administering medications and patient monitoring. • Ability to maintain professional boundaries and delegate tasks and ensure objectives are achieved. With my past experience as a senior staff nurse level three, I maintain therapeutic relationships and able to maintain good effective working relationships with patients, carers, staff and families. I am very reliable and work well in a busy environment and work well independently. Besides being on the floor most of the time I do appraisals for junior nurses and care assistance. • Knowledge of patient’s care plans and reviews as required. • I manage the phases of the nurse-patient relationship ensuring that patients are treated with dignity and respected as individuals and given chance to participate in their own care planning. • I have skills in peg feeding and Tracheotomies. I am able to care for patients with such as anorexia. Currently I work in hospital wards and A&E looking after patients who need specialised 1:1 care 24hrs a day. The main purpose of my role is to keep the patients safe ensuring that they are taking their medications as prescribed, ensuring their safety whilst communicating with them effectively and building a rapport with them to enable to express their thoughts and feelings. In summery I have knowledge in legal matters pertaining to CARE QUALITY COMMISSION, MENTAL HEALTH ACT (2005)2007), NURSING AND MIDWIFERY COUNCIL (CQC, NMHA, NMC, SOVA) SAFEGURDING OF VULNERABLE ADULTS.
