I am currently practising as an RMN, supporting individuals with a range of mental health conditions, including dementia, while delivering high-quality, person-centered care. During my time at District Hospital (SDH), I worked across multiple wards and specialities, including stroke units, surgical wards, endoscopy, hospice care, the emergency department (ED), Acute Medical Unit (AMU), and many other clinical settings. This broad experience enabled me to develop a strong understanding of holistic patient care and adapt effectively to different clinical environments. This included me looking after learning disability and confused patients with patience. As a Nursing Assistant, I provided high-quality physical, emotional, and personal care to patients with diverse medical needs. My responsibilities included assisting patients with activities of daily living such as personal hygiene, washing, dressing, toileting, feeding, mobility support, and maintaining dignity and independence. I monitored and recorded vital signs, including blood pressure, pulse, temperature, oxygen saturation, respiration rates, and blood glucose levels where appropriate. My experience as a Support Worker with AWP inspired and motivated me to pursue further education in mental health nursing. Through dedication and determination, I completed my studies and qualified as a Registered Mental Health Nurse (RMN). As a Registered Mental Health Nurse, I am responsible for providing comprehensive mental health assessment, treatment, and person-centered care to patients experiencing a wide range of mental health conditions, including dementia and complex mental health presentations. My role includes conducting detailed mental health and risk assessments, developing, implementing, and evaluating care plans, supporting recovery-focused interventions, and promoting patient safety and wellbeing. I am experienced in medication management, including the safe administration and dispensing of medication, monitoring therapeutic effectiveness, identifying side effects, and ensuring medicines are administered in accordance with professional guidelines and legal frameworks. I work collaboratively with multidisciplinary teams to coordinate care and advocate for patients and their families. Furthermore, my role involves assessing mental capacity, managing risk, maintaining accurate clinical documentation, supporting crisis intervention, safeguarding vulnerable individuals, and providing psychoeducation to patients and carers. I regularly use therapeutic communication skills to build trusting relationships, support emotional wellbeing, and promote recovery and independence.
