My most recent Job with Home care, as a Learning Disability Support Worker helped me gain more experience because i was able to work in various locations within the agency. I worked with the Forensic unit where i had to be in the presence of patients struggling with mental health, Learning disability and substance misuse issues, my role was to demonstrate empathy, understanding, and a non-judgmental attitude, creating a safe and supportive environment for patients to express their feelings and concerns. I also closely observed and monitored the Vitals, behavior, mood, and changes in patients' mental health. I also reported Accurately and in details any significant observations on RIO and E-Obs. Furthermore, I also Collaborated with the care team, such as psychiatrists, psychologists, therapists, and social workers. Sharing relevant information, attending team meetings, and actively participating in discussions that are crucial to provide comprehensive care and support for patients. I understood the importance of Maintaining accurate and up-to-date records for continuity of care, hence i always Updated the Datix form when there is an incident, and also filled the incident report form in details. As a Recovery worker, I understand how essential it is to handle crisis prevention and intervention. I am trained to recognize signs of distress and can provide immediate support during difficult times. By empowering individuals with coping strategies and crisis management techniques, contribute to reducing the risk of relapse and hospitalization.
