Frequently asked questions for managed care

Curam have provided 5 million hours of care support to clients with a wide range of needs and conditions. Some clients require a 'managed care' solution in which the Curam team works with the service user to define the requirements so that a clear care plan can be agreed and shared with Care Professionals.

Managed care refers to services within the care and health pathway delivery with the aim to manage the quality, cost, and utilization of services. It involves coordinated care plans that ensure individuals receive appropriate and timely care.

Any Adult who requires support, with physical or learning disabilities, mental health issues, chronic illnesses or general care management services may be eligible for managed care services.

A care support plan is a document that outlines the type and level of care you will need. It’s completed based on our person-centered approach to understanding your service needs and overseen by the Registered Care Manager. It covers biographical details to ensure we match a suitable Care Professional. It usually has input from other professions such as a social worker or nurse. The conditions presently experienced by the service user and the stipulated care requirements are described to ensure the Care Professional addresses these needs. There will also be preferences for social and well-being requirements to address the 'whole person' and not simply the tasks related to the conditions.

Where the care package is regulated the care support plan will be accessible to the Care Quality Commission [CQC] if required. Privacy & Confidentiality will be maintained at all times.

Within the Care Support Plan, the care professional will record notes and data to the care record. This may be shared with agreed other parties. These records will also be accessible to the CQC where the care is regulated.

A home assessment is conducted at the outset of the care package. It can be done remotely initially but will be completed / confirmed when the first home visit occurs. The assessment is designed to check if home-based issues may affect the care requirement. This may relate to handrails or loose carpets. It may relate to where the toilet / bathroom is.

If there are issues identified, they will be discussed with the client and the registered manager. Some outcomes may be the request for handrails or a stairlift or other changes.

The risk assessment is usually done at the same time as the home assessment. The purpose here is to assess if there are risks associated with the care package requirements and the conditions in the home and with the service user. An obvious aspect is the mobility / weight of the service user and their need for manual support in moving around or using the toilet or getting into or out of bed.

Where risks are identified the care package may be edited or referred back to the social worker.

Care packages will be a service normally carried out over time stretching for weeks / months ahead based on the service users care requirements. Depending on the level of care requirement there may be a need for multiple care professionals to support the service user. Their attendance hours must be planned to ensure an acceptable level of continuity. In addition, as care professionals, like any other worker, may be ill or on leave we would ensure cover is provided.

A schedule of care visits would be agreed at the outset to allow the service user to be clear and confident of the support being provided.

All Curam carer professionals must have a valid DBS which we check at the induction / onboarding stage. No care professional is approved without this.

DBS’s have time limits at issue and require renewal. We notify care professionals in advance if their renewal date is coming up. Sometimes, we may need to stand down a care professional who has not renewed for some reason.

All care professionals must be security checked in order to be accepted on the Curam platform. As with other workers the right to work and appropriate evidence of skill are a mandatory requirement. We also check references for the managed care service.

The normal service is available from 08.00am until 18.00pm.

Any calls between 18.00pm-22.00pm will be answered by our call answering service. they will take a note of the details and direct the message to one of our teams internally.

After 22.00pm and up to 08.00am, you can leave a message that will be picked up the next working day.

If your call is urgent but not an emergency, but you feel requires prompt attention please call the NHS emergency number on 111.

If you have a managed care service agreement then your Care Support Plan will contain details of the contact numbers in the event of the need to escalate a concern or request.

In all other circumstances please leave a brief message stating your name and contact details plus a clear summary of the issue or request. Your message will be picked up on the next working day and someone will respond.

HV tracking is used by some service users to monitor their health vitals. This will normally be related to care plans and the requirement to share the data with nominated professionals or family members.

You can talk with the Managed Care or Live-in teams about this if you wish to understand more or include it in your service plan.

The method of capturing will be determined by the care support plan or agreed with the client and the care professional.

For regulated care packages the care plans & related information will be accessible to CQC during any audit reviews . Privacy & Confidentiality is always maintained.

Curam is rated as ‘Good’ with the CQC.

Some clients will request sharing of data with nominated other parties. For Curam that will be captured in the care support plan. Usually that will be the social worker, the GP and perhaps other nominated clinicians such as hospital consultants.

Curam does not share personal data with other parties. We follow GDPR and Data Protection Act rules and standards.

The Registered Manager is the approved qualified person who oversees the regulated care packages managed by Curam and is registered with CQC for all regulatory reviews and compliance for the services delivered. Any CQC requests for information on regulated services is managed by the Registered Manager. The manager is responsible for ensuring care packages are appropriately set up, administered and delivered using our pool of approved care professionals assigned to regulated care packages.

The Managed Care team will help you define your care requirements. You can apply for support from the Local Authority and their adult care team can assist in defining your requirements, which you can use to refer to us.

Managed care services can include personal care (e.g. help with eating, washing, dressing), domestic support (e.g. cleaning, shopping), and health-related services (e.g. medication administration, therapy).

Managed care can be funded through a combination of public funding, personal contributions, and sometimes private insurance.

Yes, you have the right to choose your care provider.

Care plans are typically reviewed annually, but you can request a review at any time if your needs change.

If you are not satisfied with your care, you should first discuss your concerns with your care provider.
