Support clients to become involved in the development and review of their support plan; encourage to take advantage of opportunities to be consulted and involved in matters relating to the management of their property (if appropriate) or other matters affecting them. Encourage and support service users to take an active part in activities within their community.
Carry out client reviews via support plans and risk assessments.
Support clients to maintain their own home, even in periods of instability, and liaise with other agencies to help ensure this happen
After 18.00 pm I charge £30.00 per hour due to unsociable hours.
