I ensure the delivery of quality of care by continually assessing the needs and wishes of my clients. Based on my exceptional high standard in person centred care, I developed, reviewed, and updated care plans to meet my clients changing physical, social, and psychological needs. I can deliver high quality short-term reablement services within specified timeframes. I have successfully achieved time-bound outcomes in various projects, including those with a 6-week duration. I am experienced in personal grooming, and I enjoy helping clients look their best. I can assist with hygiene, toileting, eating, cooking, cleaning and any administrative tasks which need support. To demonstrate that I am delivering high-quality care that is responsive to individuals’ needs and preferences, I use outcome measures that assess factors such as satisfaction, safety, and quality of life. I also collect feedback from clients and their families to ensure that I am meeting their expectations. Ongoing training is a priority, I undergo regular refresher courses, as well as specialist training to handle more complex care situations.
