I carefully observe their care plan and ensure that the people we support are in safe and clean environment, I provide physical and personal care (changing of pads if the people we support are incontinent) bathing, preparation of meals, doing the laundry where necessary. I can support individuals out in the community, I can draw person cantered activity planner suitable around individual interest, wishes and preferences. I can support individual to attend appointment and connect with other professionals. I support individuals to book and attend medical other appointments, and social engagement I ensure that the people we support are given equal opportunity and are treated based on individual needs while also considering cultural and dietary needs, e.g., halal meat preferred by Muslims, vegetarian does not eat meat, and kosher preferred by Jews. I understand that people has differing needs and providing service that meets these various needs is essential. I give teamwork an important place as it device efforts and multiply effect. I receive supervision at the organisational schedule times to discuss work performance, air my own contributions towards improving service provided the people we support/ own development and to get guidance/clarity on difficult issues. Communication is an important tool in carrying out daily task, it allows me to professionally engage with the people we support, their family, other members of staff, senior colleagues and other professionals. I use individuals preferred language while engaging with individuals. I can develop a pictorial communication tool (little book, poster, little album, cards) for individual who best engage/express their choices in this way. I can write report and provide individual case note, I am responsible for completing the daily log at the end of every shift and leaving messages in communication book. I ensure all entries are clear can be understood and are directed towards the right person and purpose. My service to residents ranges from maintenance of safe clean and comfortable environment on resuming shift, ensuring fire doors are shut, exit doors are free from obstruction and environment free from hazards. I ensure medication are locked away in safe cupboards, I can duly prompt and or administer medication. I ensure all COSHH are correctly stored away in locked cupboards. Ensure adequate record keeping plus provide record on event and activities. Writing incident and accident report where situation demands while Maintaining confidentiality
