If there is a care plan in place, I follow the individual care plan, if not then I have to sit with the family the client and make a care plan that suits the client depending on their needs. I also reflect on my day today's work to improve work practices, liaising with family, nurses and doctors to fulfill the clients’ needs. I report to the family and other people involved all changes and wellbeing of the clients. Also, work as a team with other professionals who come in to support the clients in a centered way and ensure all personal hygiene aspects of the clients are met. This may include hoisted if needed, washing, cooking, and feeding, changing pads depending on each individual needs. I also have a duty of empowering service users and treating each person as an Individual. Having worked in health and social care for more than a decade, I have been pursuing not only to be a caregiver but to assess and provide a client centered assessment and intervention strategies and deliver unique support options for them and be able to determine the needs and goals of the clients.  HD7 6HG 280925 I’m Nancy, an experienced P/A currently living in West Yorkshire. I do specialise as a Palliative and End of Life Personal Assistant/Carer and as a community End of Life Doula but if you’re not palliative or end of life don’t let that put you off exploring if I might be the right person to meet your needs as I have a wealth of transferable skills and always open to learning new ones! I’ll also add to that to not look at my age and think “nah, she’ll be too old”. I’m healthy, enjoy running, walking, wild swimming and camping and am always up for making the most of life. Working in the end of life field is not all doom and gloom you know. Most people want to live fully right up until the last breath and that includes getting in as much laughter as possible so you could say I’m well versed in engaging with life even in the quiet moments which I love too. So, as well as supporting people in having their wish to die at home fulfilled as practicably as possible I also support people in living at home in a variety of ways. I'm here to alleviate some of the stresses of living independently and as fully as you can and make the time more about quality and less overwhelming. Now on to my End of Life Doula explanation. As an End of life Doula I serve as a non-medical practitioner, supporting you and your loved ones as you navigate your end of life experience. I make myself available to assist a dying individual and, typically, the family before, during, and after a death occurs in order to provide physical, emotional, psychological, and even spiritual support. I complement systems already in place, allowing caregivers to feel well supported and those living remotely the assurance that their loved-one is well supported. You can bring an End of Life Doula on board whether you are being cared for at home, care home, hospice or hospital. First and foremost an End of Life Doula is a grounding, reliable, knowledgeable, comforting and reassuring presence during an emotional time. I provide knowledge of the death process and a compassionate resource for families who may be overwhelmed and grieving. I advocate for you. I will meet you where you are and assist with developing a cohesive plan which honours all who are involved. But my service as a doula goes beyond that. I’m fully insured both as a doula and PA and have an Enhanced DBS on the update service.
