I work with people with physical and/or mental health conditions and those at risk of developing them. . Health and wellbeing counselling can be an effective intervention for people experiencing a range of long term conditions, including respiratory, cardiovascular (including type 2 diabetes and hypertension), and stress/low mood. I also support people with weight management, diet, increasing activity levels, and setting personalised goals to change their behaviours. I can help you maintain your independence, autonomy, and social connection after tramua or bereavement. I Provide short term support and practical help for Curam clients recovering from disability, returning from the hospital, or convalescing at home. I help clients to organise, pesonalise, and optimize care plans that gives them control over their health and wellbeing, I achieve this by sign posting and social prescribing, supporting exercise and physiotherapy, connecting clients with the local community and voluntary groups offering a range of activities and services.. I also help with or advise on challanging administative tasks such as completing benefits applications and appeals, finding employment, well-being, diet amd exercise, legal issues, complaints, mental health tribunals, self-directed care assessments, organising travel, letter writing, funeral planning, Windrush Compensation Scheme applications, witnessing LPA, passport pictures. ..etc .  I help identify what is affecting client's ’s health and wellbeing. Then, once we set achievable goals, I get to work to find services and activities that offer practical, social, and emotional support that’ll improve the client's health and wellbeing. Connecting clients with local community activities and services. I also provide support to tackle issues that can affect the health of clients by working with integrative care practitioners and NHS healthcare professionals who provide more traditional social care and medical interventions.   I will usually work with a client over several sessions, depending on their needs. I also offer combined companionship care, which is flexible and friendly. I am a good listener and I can undertake both administrative and practical tasks such as paperwork and administrative tasks, light housework, cooking, laundry, shopping, dog walking, picking up items, delivery, running errands, and or helpi client with diet, nutrition and exercise, walking and travel. I have trained in counselling and life coaching and information, advice & guidance (IAG) to help clients with resolving health, social care, employment, and legal issues. My main experience in care is in supporting the elderly. Helping people with dementia, depression or other mental challenges, to set goals and create systems to help maintain their independence. I am also an advocate, with over 30 years of experience in support, advice, and legal guidance. i basically offer a package of extracurricular support that enhances the client's care package and improves long-term health and recovery outcomes. I am flexible and available online, or one to one home visits, to clients in my local area.
