I have worked in care with individuals with many conditions including dementia and I have developed a good understanding of the emotional needs and the concerns that come with being afraid and vulnerable due to this condition. I am an NMC registered adult nurse with 3.5 years of post registration experience. Though still very new in nursing I have experience of working in acute medicine, elderly mentally infirm patients and various bank shifts across the trust. I have an excellent relationship with the wider multi disciplinary team including consultants, GPs, occupational therapists, physiotherapists, discharge coordinators and district nurses. For example, nursing a deteriorating patient in a nursing home setting where there was no core drugs and no preferred priority of care. I was able to liaise with the GP and arrange emergency core drugs and arrange for preferred priority to be put in place meaning the patient passed peacefully in there current setting. I have excellent communication skills including extremely busy and demanding environments, patients with communication difficulties and patients from a variety of social and cultural backgrounds. For example when nursing a young patient with brain stem death, awaiting the transplant team I was able to support the patients parents throughout the process, helping them to find accommodation and keeping them updated via regular phone conversation. I received a card thanking me for my empathy and compassion as well as a letter from the transplant team thanking me for all of my help. I also ensure all written communication is clear, accurate and professional. I actively promote the health and well being of clients offering advice where possible and signposting or referring patients to other services where needed. I aim to always maintain holistic care and understand nursing is not only about the physical condition of a client , but the need for treating each client as an individual, supporting clinical governance in safe, effective and person centred care. I am commited to continuing professional development and have attended courses and study days relevant to my role. For example, with a shortage of learning disability nurses I attended an NHS lead course on communication, struggles patients with learning disabilities can face when in hospital and changes that could be made to help support the learning disability community. I always ensure mandatory training is up to date and I use online resources to keep informed of nursing initiatives. I am a very positive person and I have a positive attitude which enables me to build good relationships with clients , relatives and colleagues. I am a compassionate person and pride myself on being empathetic and understanding with my clients.
