TEAM LEADER Healthcare Assistant / Support Worker Team Leader responsible to ensure smooth running of the unit at all times. Working directly with clinical teams London & Richmond Borough. Spending time with the Service Users discussing their interests and dislikes and how their needs can be addressed better. Supporting the team with Personal Care and conducting Indoor and outdoor activities when required. Compiling weekly and monthly reports and liaising with Senior Management on clients continuing care. Responsibilities: • Responsible for day to day running of home, maintaining occupancy level and delivering a consistent high standard of care • Support individuals with mental health problems, learning disabilities within supported housing setting to lead fulfilling lives, those attending Day Care via the provision of support, assistance and personal care. • Contribute to an effective service by understanding the rights and needs of people with mental health problems, learning disabilities and other needs, which require care and support.
