TEAM LEADER Healthcare Assistant / Support Worker​​​​ Team Leader responsible to ensure smooth running of the unit at all times. Working directly with clinical teams London & Richmond Borough. Spending time with the Service Users discussing their interests and dislikes and how their needs can be addressed better. Supporting the team with Personal Care and conducting Indoor and outdoor activities when required. Compiling weekly and monthly reports and liaising with Senior Management on clients continuing care. Responsibilities: • Responsible for day to day running of home, maintaining occupancy level and delivering a consistent high standard of care • Support individuals with mental health problems, learning disabilities within supported housing setting to lead fulfilling lives, those attending Day Care via the provision of support, assistance and personal care. • Contribute to an effective service by understanding the rights and needs of people with mental health problems, learning disabilities and other needs, which require care and support. Valuing the rights of people with a disability. • Ensure that the Care Quality Commission (CQC) and the Health and Social Care Act 2008, Chiltern Support & Housing Policies and Procedures and any other Legislation maintain the highest standards of care in accordance with the Essential standards as set out. • Maintain Facilities to high standard and Comply with Health & Safety requirements. • Procurement of Supplies and Budget control • Encourage service users to make informed choices • Support service user participation in activities • Enable and assist service users to access a varied range of leisure activities and services in the local community • Encourage and assist service users to look after their own finances • Participate and assist service users in cooking, shopping, cleaning, gardening, laundry, ironing and general household tasks • Assist in the provision of the highest possible standards with regard to quality of life and a supported living • / • environment, including support to those users who may present as challenging in the community or within the service • Participate in ac0hieving the highest domestic standards with respect to cleaning and hygiene and support in the preparation of meals for well balanced diets that meet the cultural, diversity and dietary needs of the service users • Responsible for Residents Health & Welfare • Assist service users to have access to and receive appropriate health care • Provide personal care for service users as appropriate depending on their support needs, administering medication, handling (in accordance with health and safety guidelines) • Undertake Rota, sleep-ins and participation in and out of hours on call systems. • Attend meetings with professionals regarding service user and support service user as advised by Doctors, Psychiatrists, Psychologists and other professionals involved in the service user’s service. • Take part in important decisions regarding the service user • Ensure Staff are well trained and supported in all aspects of their role and train new support workers in working with service user. • nurse they have read all relevant paperwork, have an understanding of the service user’s needs, likes/dislikes and challenging behaviour and are aware/comfortable with the paperwork and tasks involved in working with this service user. • Liaise with Behavioural Intervention Worker to write Risk Assessments, Care Plans and Behaviour plans • Maintain records and prepare team leader reports to present to the unit manager • People management, development, discipline, supervision and appraisal • Conduct Care planning and Resident reviews and promote the company • Handle complaints and concerns timely & professionally Training Completed • Deprivation of Liberty Safeguards • Diversity and Equality • Fire Training • Food Hygiene • Health and Safety • Infection Control • Nutrition and Diet • Person Centred Care • Principles of Care and Confidentiality • Record Keeping • Risk Assessment • Role of the Care Worker • Safe Administration of Medicines • Safeguarding of Vulnerable Adults • Supervision / Appraisal Key Skills • Leadership and Management • Maintaining Records and Report Writing • Presentation Skills • Professional Working Ethic • Bi-lingual (*British Sign Language) • Coaching and Mentoring • Teamwork • Excellent Communication Skills • Excellent IT Skills (Word, PowerPoint.
