I have dealt with challenging behaviors from all aspects of complex care Dealt with all housing needs and budgeting Provide emotional support to individuals and their families. Provide practical support for individuals and their families, such as helping with household tasks, bills, finances, personal care, and general paperwork. Support and monitor their healthcare needs, including administering medication and temperature checks. Completed care plans that involve physio and hydro therapy and every day to day activities Support towards hobbies and interests. Supporting the learning of new skills or gaining employment. Teaching life skills, such as budgeting and paying bills. Helping with everyday tasks such as meal preparation, writing a shopping list, going shopping, or maintaining their housing tenancy. Helping to access community facilities and be included in community groups. Understanding communication needs and adapting communication to each individual, e.g., sounds, tones, body language. Working with other professionals, such as doctors, therapists, solicitors, social workers, etc, to ensure consistency of support. Look into the individual’s needs and draw up a focused support plan. Becoming both a role model and companion to the individual and uncovering shared interests, such as hobbies and outings. Act as key worker to named clients; regularly review their progress, set and monitor targets, and help them to acquire daily living skills Draw up guidelines as a keyworker for the induvial Support clients to become involved in the development and review of their support plan; encourage to take advantage of opportunities to be consulted and involved in matters relating to the management of their property (if appropriate) or other matters affecting them. Encourage and support service users to take an active part in activities within their community. Carry out client reviews via support plans and risk assessments. Support clients to maintain their own home, even in periods of instability, and liaise with other agencies to help ensure this happen After 18.00 pm I charge £30.00 per hour due to unsociable hours.
