These have been Nursing homes with elderly Dementia patients, people with mental health illness, young adults with severe autism, learning disabilities and challenging behaviours. Working in small teams of four to ten, I am responsible for ensuring that the people we support develop, and live independent lives, access to the community, personal care, using person centered planning approach. The duties included provision of care and support while promoting positive and effective communication with all co-workers and service users, and this has been done in close collaboration with other health care professionals such as The Care Quality commission, doctors, Physiotherapists, Psychologists, police and other approved agencies, the parents and relatives, as well as a blend of care support team at each establishments I am sent to work. I have been actively involved in providing care and life support with a person centered planning approach and ensuring that the service users adopt to live independent lives, care and support for young Adults with learning disabilities, record daily night duties, ensuring that service users gets quality sleep, tidying up their Bed rooms, offer personal care, do risk assessments as I go along, reporting to service manager, prepare daily and incident reports for the people I support. Demonstrated excellent team working skills in a highly pressured environment, such as ability to listen to my managers, and senior care support workers perform my role to a high level and support colleagues, resulting in gaining experience in the care sector. Before I gained employment at these agencies, I worked in a range of care setting, nursing home, mental Health establishments, domiciliary care support; hospitals thereby achieve high levels of experience and skills in the health industry. Now am beginning to take a course in Health and social care level 5 for future job of care and support
