ME1 2XS 288479 I feel I have the experience, knowledge and the passion for the role of care worker/ Support worker and would thrive well given the opportunity to make valuable contributions to individuals who would need my support. My personal goal and aspiration in all my roles within the health and social care is to support individuals to live a safe and quality of life and to work in a way that supports them to reach their true potential and goals. I have worked as a care assistant, supported worker and later Managed 3 residential provisions registered with CQC for adults with learning disabilities. I have also worked in the role of Care Education and treatment review Lead for over a year and could consider myself to have played my role well by supporting the various commissioners of the 6 Boroughs across South East of London with the prevention strategy of young people and children with autism and or learning disabilities being admitted into hospitals. My main role as the CETR Lead, has been to work with a range of stakeholders, NHS, private and voluntary sector providers, Continuing Healthcare and Children’s Continuing Care, Local Authorities (Children’s social care and SEND Education), NHS England, people with learning disabilities/autistic spectrum disorders/ mental health problems, their families and experts by experience to provide timely and robust support to avoid inappropriate in-patient admission to enable the child or young person to live a safe, fulfilling life at home or in the community. I also work with commissioners, staff and other agencies to ensure the suitability of services and provision of care to meet complex health needs to ensure appropriate use of NHS resources. I have successfully gained lots of experience in operating in complex, multi-disciplinary systems and managing risks.  I have conducted very complex CETR’s both in an inpatient unit and in the community. I have been part of making very strategic decisions and recommendations which have been success stories for children and young people who were at risk of being admitted in hospital but got the support and services needed to prevent this happening, making them safe and happy in their homes and community. I have had the experience of complex diary management, working in complex, multidisciplinary environments and working to tight deadlines and managing own time. I do have the ability to draft correspondence and seeking guidance as required and able to deal with individuals at all levels and in challenging situations. I also work in a charity youth organisation voluntarily as a youth mentor which has provided me with opportunities to work with young people and their families from different backgrounds giving me the opportunity in counselling and advisory setting.  The mentorship places emphasis on helping to develop personal and positive rapport with the young people and their families and establish their personal goals and help them create life action plans.  I act as a cheerleader, coach and advocate to my young people who do need such support and sign post them to resources in the community which may be beneficial to them and their goals. Moreover, this is 3 / 3 similar in mental health as there may be times that the young person in my group may be in crises and may need resources and additional support with dealing with their challenges. As their mentor, it is vital for me to be aware of the signs indicating the individuals changes of needs and presentation.  I feel that this role has provided me with a wealth of experience in supporting young people and preventing them further deteriorating in their physical and mental health. I am highly organised and do have good time managed skills with excellent attention to detail.  I also can adapt to changing programme requirements. I have acquired tactful, discreet, and diplomatic ways of working and very am very flexible and a good team player. I am looking to create regular clients, work towards knowing them, creating a professional bond to assist in meeting their needs. I enjoy cooking and am happy to assist with very balanced and nutritious meals for my clients, listening to their preference and diary needs. I can do cleaning, I.e. vacuum cleaning, washing, making beds, ironing, mopping etc when required to do so. I am happy to have cats and pets at clients residence and can feed them. Minimum hours for day care is 4 hrs per shift . Hello my name is Nicola and i have been a kind, caring, conscientious and considerate carer/support worker for many years putting the welfare of clients first and enjoying the sociable nature of this type of work, meeting new people and consider it a privilege to support and help others. I can offer a care/ companionship role with clients who are mainly elderly and frail and who may still want to enjoy a full life of their choosing. Equally, i am just as happy to support people who are more home bound and less active but might appreciate bringing activities to the home to enhance the quality of their lives, such as art and crafts, flower arranging, cookery etc. Gardening and caring for house plants is often very beneficial and rewarding hobbies/activities. Or listening to others life stories . I am willing and able to offer personal caring skills in a respectful manner, if needed and i appreciate that a persons health care needs may fluctuate from day to day. Some people may be vulnerable to certain types of health issues which need to be noticed and acted upon such as UTI's ,or constipation which can cause sudden changes in behaviour both psychological and physical therefore I also believe that these things can be avoided by good care management using relevant skills and knowledge. Assisting clients in a respectful, warm and understanding manner is important to me and i can offer a wide range of experience and a comprehensive skills. I am experienced in working awake night duties, supporting clients to maintain their night time / morning routines and any care needs which could include assisting with washing and dressing, accessing the toilet facilities during the night, support with any pain relief , falls prevention, snack or drinking needs which they may require during the night in order to help settle them back to bed for sleep or rest. Pressure sore prevention . Companionship and reassurance. Generally being available & close by should assistance be required. Sleep Over Care _ 9pm to 9am hours negotiable. . This type of care can provide that extra sense of night time safety & security especially for those who may be feeling vulnerable or wanting that extra bit of night time comforting presence within their own home . Sleep over care can be useful to provide assistance with morning and evening routines helping you to feel ready and prepared for the day ahead and then again , at night, quietly settled with a sleep promoting evening routine, safe in the knowledge that someone is close by should any need arise. . Sleep over cover can be from 1 sleep over per week up to 7 again shared with one or two other carers if required. I have enjoyed working within Health and Social Care settings for over 20 years within a variety of settings and taking on various roles from basic care work to managerial taking responsibility for teams of care workers/support staff and overseeing the welfare of vulnerable adults. Some of the duties have included care planning, manual handling assessments, medication rounds, leasing with GP's , Nurses and other primary health care professionals such as OT"s , Physiotherapists, Care Managers, Dentists, Opticians , Audiology, Chiropody, Dieticians and any other specialist health care providers who's input can all help towards ensuring a good quality of life for the person in need of help or support . I also have past experience working within range of mental & social health care settings ( NHS and Private Hospital, Residential Social Services & Private Care Homes , Charity Sectors ) which has always given me a good understanding of the psychological aspect of caring for others who may be in distress or struggling for one reason or another with aspects of their psychological and emotional well-being. I have also very much enjoyed 24 hour shifts supporting adults with mild learning disabilities who are living in small group residential homes and some larger residential homes as well as this I also have also had experience supporting people within independent living schemes. I have also experience of managing and planning day care activities for older persons as well as initiating arts and crafts activities for other vulnerable client groups as part of my role as a support worker. I also helped set up and run a monthly carers support group for relatives caring for loved ones as this role can become isolating for carers. I enjoy working in accordance with the needs and wishes of the client and I am very happy to work within their own routines and come from a person centred perspective putting a clients needs first. I am also available to provide respite care for regular carers breaks or fill in for periods of sickness. I am happy to provide short term home from hospital live in care as well or to provide live in support at those times when extra input may be needed for the short term such as during short term illness. I am a non smoker, rarely consume alcohol . I enjoy cooking healthy and nutritious meals in accordance with clients needs and preferences, am happy to plan healthy, freshly cooked meals. My personal interests are enjoying nature, walking, drawing and painting , photography mainly nature during my walks, yoga , visiting cafes for coffee and cake , gardening , flower arranging, caring for pets , enjoying observing , listening to and providing food for the garden birds. I am a member of the Sussex Wildlife Trust. I AM FOND OF PETS BUT DUE TO ALLERGY UNABLE TO WORK WITH INDOOR FELINE'S. I would be happy to carry out a preliminary visit to work out with you an individual care plan taking fully into account your routines, needs and wishes before any placement begins. This also gives you a chance to meet me and decide if you feel that we would be a good working match for each other and your situation. How much support you may or may not need with everyday activities of living such as washing and dressing, pain management, mobility , eating and drinking, your likes and dislikes in terms of daily/weekly meal planning. In fact everything that you do through out the day or any needs you have such as medication reminders, physical exercise such as short walks outside or places you like to visit. CT19 5EL 278604 Personal Statement With over two decades of dedicated experience in health and social care, and a robust academic background, I am currently pursuing a Master’s in Positive Behaviour Support to further my commitment to enhancing the quality of life for individuals with intellectual and developmental disabilities. My journey has been driven by a deep-seated passion for creating supportive environments that foster positive behaviours and personal growth. Professional Journey and Experience Over the past 22 years, I have worked across various roles in health and social care, where I have honed my skills in supporting individuals with diverse needs. My career began as a care assistant, where I obtained an NVQ Level 2 in Care, which laid the foundation for my understanding of the principles of compassionate and person-centred care. This early experience ignited my passion for working with individuals with intellectual and developmental disabilities, and it set the stage for my subsequent professional and academic pursuits. Throughout my career, I have held various positions, each adding to my expertise in the field. From working in direct care roles to taking on supervisory and leadership responsibilities, I have developed a comprehensive understanding of the challenges faced by individuals with behavioural needs and the strategies required to support them effectively. My experience spans residential care, community support, and educational settings, providing me with a diverse perspective on the implementation of positive behaviour strategies. Academic Background and Professional Development I hold a degree in Intellectual and Developmental Studies, which provided me with a solid academic foundation in understanding the complexities of intellectual disabilities and developmental disorders. My studies have equipped me with a thorough understanding of the theoretical frameworks and evidence-based practices that underpin effective support strategies. Complementing this, I earned a Diploma in Health Studies, further deepening my knowledge of health and social care principles. As I progressed in my career, I recognized the need for continuous professional development to stay abreast of the latest advancements in the field. I have participated in numerous training programs and workshops, including specialized courses in positive behaviour support and behaviour analysis. These opportunities have allowed me to expand my skill set and apply cutting-edge techniques to my practice. I look forward to hearing from you to discuss how I can best assist you in your home. TN23 3GL 45885 I am a friendly and driven individual who really enjoys my job and who consistently aims to put my best efforts into everything I do. I am constantly reminded of how fulfilling my job is when I meet individuals and families who I can make a difference to their lives. Naturally, I always find myself doing whatever it takes to ensure the person I am assisting is not only comfortable but also happy in general. I believe that I have a lot of skills to offer, being a younger person too. I have been in care for 4 years and have worked for two agencies during that time. This has meant that over this period of time, I have worked with across multiple homes and with hundreds of individuals allowing me to be able to gain a huge amount of experience as well as mastering the skill of adapting to new environments constantly. I also am very energetic and have a passion for cooking and reading. I have just finished my undergraduate degree in English literature and now plan to take some time out to work before going on to do a masters. I have a lot of free time to offer and can be called on short notice too which I feel is a huge advantage for a client. I am happy to answer any questions that one may have before you committing to having me as a career! {"field_week_day_first":1,"field_is_available_first":false,"field_care_type_first":"","field_week_day_second":2,"field_is_available_second":false,"field_care_type_second":"","field_week_day_third":3,"field_is_available_third":false,"field_care_type_third":"","field_week_day_fourth":4,"field_is_available_fourth":false,"field_care_type_fourth":"","field_week_day_fifth":5,"field_is_available_fifth":false,"field_care_type_fifth":"","field_week_day_sixth":6,"field_is_available_six":false,"field_care_type_sixth":"","field_week_day_seven":7,"field_is_available_seven":false,"field_care_type_seven":""}
