I can work well as a team as well as an individual to complete tasks. I can communicate effectively in English, and I listen to others to meet their needs. My healthcare journey began at District Hospital (SDH), where I worked as a Nursing Assistant for more than seven years, gaining experience across a wide range of clinical wards and departments. My passion for patient care, combined with my commitment to continuous professional development, motivated me to further my studies and qualify as a Registered Mental Health Nurse (RMN). I am currently practising as an RMN, supporting individuals with a range of mental health conditions, including dementia, while delivering high-quality, person-centered care. During my time at District Hospital (SDH), I worked across multiple wards and specialities, including stroke units, surgical wards, endoscopy, hospice care, the emergency department (ED), Acute Medical Unit (AMU), and many other clinical settings. This broad experience enabled me to develop a strong understanding of holistic patient care and adapt effectively to different clinical environments. This included me looking after learning disability and confused patients with patience. As a Nursing Assistant, I provided high-quality physical, emotional, and personal care to patients with diverse medical needs. My responsibilities included assisting patients with activities of daily living such as personal hygiene, washing, dressing, toileting, feeding, mobility support, and maintaining dignity and independence. I monitored and recorded vital signs, including blood pressure, pulse, temperature, oxygen saturation, respiration rates, and blood glucose levels where appropriate. I supported infection prevention and control measures, assisted with moving and handling procedures, and worked closely with multidisciplinary teams to ensure patients received safe and effective care. Additionally, I supported patients during admissions and discharges in A&E, AMU and other wards, documented patient observations, escalated concerns regarding deterioration in physical or mental health, and assisted qualified nurses and medical teams during procedures and treatments. Working in specialist areas such as stroke, surgical and hospice care enhanced my ability to provide compassionate, patient-centred care in both acute and end-of-life settings, while experience in the Emergency Department and AMU strengthened my ability to work under pressure, prioritise workload, and respond effectively in fast-paced clinical environments. Following my experience at SDH, I progressed to working with Mental Health Partnership NHS Trust (AWP) as a support worker. This role provided me with invaluable insight into mental health care and strengthened my passion for supporting individuals experiencing psychological and emotional difficulties. As a Mental Health Support Worker within AWP, I supported patients with a variety of mental health conditions by promoting recovery, independence, dignity, and emotional wellbeing. My role involved engaging patients in therapeutic activities, providing emotional reassurance, supporting patients with daily living activities, maintaining accurate records, and observing and reporting changes in mental state, behaviour, and risk. I worked closely with multidisciplinary teams, including nurses, psychiatrists, psychologists, occupational therapists, and social workers, to support care planning and patient recovery. I also supported patients experiencing anxiety, depression, psychosis, dementia, autism, behavioural challenges, substance misuse, and other complex mental health needs. My responsibilities included maintaining a safe therapeutic environment, undertaking risk observations, de-escalating challenging behaviours, supporting patients during periods of distress, and ensuring safeguarding procedures were followed. Through this role, I developed strong communication, empathy, resilience, risk awareness, and interpersonal skills essential for mental health practice. My experience as a Support Worker with AWP inspired and motivated me to pursue further education in mental health nursing. Through dedication and determination, I completed my studies and qualified as a Registered Mental Health Nurse (RMN). As a Registered Mental Health Nurse, I am responsible for providing comprehensive mental health assessment, treatment, and person-centered care to patients experiencing a wide range of mental health conditions, including dementia and complex mental health presentations. My role includes conducting detailed mental health and risk assessments, developing, implementing, and evaluating care plans, supporting recovery-focused interventions, and promoting patient safety and wellbeing. I am experienced in medication management, including the safe administration and dispensing of medication, monitoring therapeutic effectiveness, identifying side effects, and ensuring medicines are administered in accordance with professional guidelines and legal frameworks. I work collaboratively with multidisciplinary teams to coordinate care and advocate for patients and their families. Furthermore, my role involves assessing mental capacity, managing risk, maintaining accurate clinical documentation, supporting crisis intervention, safeguarding vulnerable individuals, and providing psychoeducation to patients and carers. I regularly use therapeutic communication skills to build trusting relationships, support emotional wellbeing, and promote recovery and independence. Working with individuals living with dementia has further enhanced my patience, compassion, and ability to deliver personalised care tailored to individual cognitive and emotional needs. Throughout my career, I have remained committed to delivering safe, compassionate, and high-quality care while continuously developing my knowledge and professional skills. My broad clinical experience across physical and mental health settings has enabled me to become a well-rounded healthcare professional with strong clinical judgement, adaptability, and a genuine passion for improving patient outcomes. I am dedicated to making a positive difference in the lives of those I support and remain committed to ongoing professional growth within healthcare.
