Once I’ve reviewed the plans, I begin my rounds. This involves checking in on each patient, assessing their physical and emotional well-being, and providing any necessary assistance with daily activities such as bathing, dressing, and meal preparation. I take the time to engage with them, listening to their concerns and ensuring they feel comfortable and supported. Throughout the day, I also administer medications, monitor vital signs, and document any changes in my patients' conditions. Communication is key, so I regularly update family members and collaborate with other healthcare professionals to coordinate care effectively. In the afternoon, I may lead or assist with therapeutic activities, such as physical therapy or social engagement programs, aimed at promoting the well-being of my patients. As my shift comes to an end, I ensure a smooth transition by briefing the next shift on any important updates or ongoing concerns.
