These responsiblities includes: -On going monitoring and observation of patients, this includes working closely with ...More In my current job as a healthcare worker for people with mental health problems, many of my key responsibilities shows a range of transferable skills that I could bring to the post. These responsiblities includes: -On going monitoring and observation of patients, this includes working closely with patients and team members to be aware of any improvement or deterioration indicating that changes to support are required or that other agencies need to be involved. -Making sure that obeservation and monitoring of patients is accurately reported and documented. -Supporting patients in the transition from hospital to home also in carrying out daily living activities. -Supporting patients to maximise income, this includes being aware of benifits system and referal to welfare rights officer if nessesary. -It is my responsibility to ensure that I am up to date with current organisational policies and procedures and that all work undertaken is in accordance with this. -I participate and contrbute to supervision with my line manager to evaluate my performance and areas of development required. I feel the above examples show that I have good communication and interpersonal skills. I have the ability to work as part of a team and on my own inititive. I can communicate verbally and in writing. I can complete assesments under supervision. I have knowledge of welfare rights. I am a motivated person and have good organisational skills. I am willing to participate in appropriate training. I am highly motivated to deliver excellent results with challenging time scales. I am flexible and very keen to acquire new skills.
